What is your rate and do you take insurance?
For a variety of reasons, I do not participate on managed care panels at this time. However, for those who have PPO coverage with out-of-network benefits, I can provide you with a superbill if you would like to attempt to for reimbursement by your managed care provider.
Do you have a reduced fee or sliding scale?
I believe quality care should be affordable so reduced fee services are available on a limited basis.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call (800) 985-3059.
What payment forms do you accept?
Cash, check and major credit cards are accepted for payment which is due at the time of service. If you intend to pay via credit card, please fill out the following credit card authorization and bring it to your first session.
What is your cancellation policy?
If you are not present for your scheduled therapy appointment and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.

The Board of Behavioral Sciences receives and responds to complaints regarding services provided within the scope of practice of (marriage and family therapists, licensed educational psychologists, clinical social workers, or professional clinical counselors). You may contact the board online at www.bbs.ca.gov, or by calling (916) 574-7830.
